One way and another, this book has been more than 30 years in the making. Even if I were to try to remember all of those people who have contributed to it, and to record what they have done, I do not think that I would come close to acknowledging all of the debts that are due. Members of my family, colleagues, teachers and students have all contributed at different times and in different ways. Some, particularly close friends and colleagues know that they have contributed, because they have read draft material and have provided helpful comments on it. Others may not even know that they have contributed, as when students have shown an interest in the issues that have interested me. That disinterested commentary on what might be good topics for inclusion has been particularly helpful.
